The term “pediatric medical subspecialist” denotes a pediatrician who has obtained sub-board certification in 1 of the 16 American Board of Pediatrics (ABP)–designated subspecialties.
Research has provided evidence of a physician undersupply in many pediatric subspecialties through a variety of indicators. According to an article by Ethan Alexander Jewett, some of these indicators focus on the supply and availability of pediatric subspecialists. The difficulty in recruiting residents into subspecialty training programs and practicing pediatric subspecialists into vacant job positions is one important indicator. Recruitment problems have been reported in pediatric dermatology, pediatric radiology, child neurology, and pediatric rheumatology, with candidate searches lasting well over a year. The National Association of Children’s Hospitals and Related Institutions (NACHRI) has reported similar recruitment problems for pediatric neurology, pediatric anesthesiology, and pediatric gastroenterology.
Even for those positions that are filled, pediatric subspecialists in fields such as child and adolescent psychiatry, pediatric otolaryngology, and pediatric surgery have identified that they are too busy to satisfy patient demand for services. In pediatric emergency medicine and pediatric critical care medicine, for example, the heavy workload and long hours have led to high stress and burnout.
However, pediatric subspecialists enjoy comfortable salaries. According to Ron Fasnacht, senior vice president and managing principal of Cejka &Co, academic hospitals are offering salaries of a minimum of $200,000 to pediatric anesthesiologists, and some are going as high as $275,000. In private practice, these physicians earn as much as $400,000. Other pediatric subspecialists are being offered comparably lofty sums. In some cases, academic institutions are setting up these doctors in a private practice at a university to lure them with competitive compensation plans.
What accounts for the new popularity of these doctors? “One reason is consumer demand, spurred by easy access to information on the Internet,” Fasnacht says. “For instance, parents with a child who needs surgery are increasingly aware of the existence of pediatric subspecialists and now insist that the surgeon and the anesthesiologist performing the operation be specially trained to treat children.” And highly competitive hospitals are eager to give those parents what they want. To cope with reduced reimbursement rates from the federal government and an oversupply of beds, many hospitals are introducing new pediatric service lines to maximize their patient censuses.
